The foundational biomotor abilities include (but not limited to): strength, endurance, speed, coordination, and flexibility. The derived biomotor abilities include (but not limited to): mobility, balance, power, and agility.

WAIVER AND ASSUMPTION OF RISK I, as the parent or legal guardian of the above-named child, do hereby grant permission for his/her participation in any and all sports performance activities organized by Dusica Dordevic and the DuTraining Tennis Performance team. I am aware that participation in the DuTraining Tennis Performance Program has some inherent risks and injury can occur. I authorize, without prior approval, the rendering of any emergency medical treatment that may become necessary due to his/her participation in the activities. I attest that my child has no known medical conditions which prohibit participation in DuTraining Tennis Performance Program. I waive and release Dusica Dordevic from any and all actions, causes of the action, damages, claims, or demands of whatever kind or nature which I or the above named child may have against her for any injuries, known or unknown, which are incurred by, arise from, or in any way relate to the above named child’s participation in the DuTraining Tennis Performance Program. I will be responsible for any medical or other charges in connection with his/her involvement in the program. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to, or infected by COVID-19 by attending the DuTraining Tennis Performance Program. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I or my child may experience or incur in connection with my child’s attendance in the DuTraining Tennis Performance Program. I understand that Dusica Dordevic is not responsible for any lost or stolen items. I grant and give Dusica Dordevic and the DuTraining Tennis Performance team the right to use my child’s photograph or image, with or without my name or the name of my child, both individually and in conjunction with other persons or objects for any and all purposes including, but not limited to, private or public presentations, advertising, publicity and promotion. I understand that a copy of this waiver of liability and disclaimer will not be provided to me unless I request it. I HEREBY ACKNOWLEDGE BY MY SIGNATURE THAT I HAVE READ, UNDERSTOOD, ACCEPTED, AND AGREED TO THIS DOCUMENT.
